About the CHR Integration Toolkit Series

The purpose of this toolkit series is to support Community Health Representative (CHR) and Tribal health system interest holders to collectively assess and improve CHR integration into health and human service systems and teams.

This toolkit series was developed by the CHRs WITH uS! initiative, which is a consortium of Arizona CHR programs, the Arizona Advisory Council on Indian Health Care, and Northern Arizona University’s Center for Community Health and Engaged Research (CHER).
