Responses are evaluated primarily based on content and insight rather than writing style alone. Authentic, thoughtful responses are valued over polished or formulaic writing. Use of generative AI tools to draft or substantially revise responses is not permitted. Applicants are expected to proofread grammar and spelling and submit clear, professional responses.
Honors, Recognition, or Notable Milestones
Please list up to five honors, awards, recognitions, certifications, or significant milestones you have achieved. These may be academic, professional, community-based, or personal (e.g., Dean’s List, workplace recognition, licensure or certifications, select scholarships or competitive awards, promotions, or other accomplishments you are proud of).
For each item, please include:
· the name or brief description of the honor or milestone
· the approximate date received (month/year or year)
· the context in which it was earned (academic, professional, community, or personal)
Not all applicants will have formal honors or awards; this section may be brief or left blank.
Engagement, Service, and Community Contribution
Describe one to three significant ways you have been involved beyond your required coursework through organizations, service, or community engagement. This may include participation in or advising student organizations, professional associations, faith-based involvement, volunteer work, mentoring, coaching, advocacy, service within schools or the broader community, or other meaningful commitments. If you have held leadership or advisory roles, please identify them.
For each example, briefly include:
· your role
· the setting or organization
· the approximate dates of involvement (month/year–month/year or “current”)
· the impact of your involvement
Focus on describing your engagement and contribution rather than just providing a list.

Employment and Professional Responsibilities
Describe one to three significant employment or professional roles you have held. Focus on experiences that were meaningful to your professional growth or preparation for work in education or behavioral health fields. For each role, briefly include:
· your role or position
· the approximate dates of involvement (e.g., month/year–month/year or “current”)

(Maximum 300 words)
Values & Identity
People in education and behavioral health fields hold different roles, but all bring values, perspectives, and commitments into their work.
Describe the values or principles that guide how you approach learning, teaching, leadership, or supporting others. If you are early in your journey, you may describe the kind of educator, educational professional, or behavioral health fields professional you hope to become. If you are already working in the field, you may reflect on how your experiences have shaped your approach.
(Max 250 words)
Additional Context (Optional)
Many students balance coursework with other responsibilities. If appropriate, briefly describe any responsibilities, challenges, or circumstances that have impacted your educational journey and how you have navigated them.
(Maximum 150 words)
