The Scholarship Reinstatement Request form is intended for students who:
- did not meet the minimum GPA requirement for their scholarship renewal,
- did not meet the minimum enrollment requirement for their scholarship to disburse for a term, and/or
- who took a leave of absence from NAU for medical or personal reasons
Do not submit this form if you were enrolled in study abroad classes and you are waiting for credits to post from your abroad institution. Instead:
- Send your unofficial transcripts showing grades to scholarships@nau.edu, and
- Send your official transcripts to the Office of the Registrar.
Do not submit this form if you were enrolled in summer classes at NAU and you are waiting for grades to post. You will be re-evaluated once summer grades are posted.
Potential Reasons for Reinstatement Request
Students may request reinstatement of a scholarship for various reasons. Common examples include, but are not limited to:
- Mental or Physical Illness or Injury: A medical condition or incident, affecting you or a close family member, that significantly impacted your ability to meet academic or scholarship requirements.
- Death of a Family Member or Significant Person: The loss of an immediate family member or someone with significant emotional importance.
- Work Schedule Conflicts: Employment obligations that interfered with your academic performance or eligibility.
- Religious Mission: Participation in a religious mission that required an extended absence from school.
- Major Life Event: A substantial personal circumstance, such as a family crisis, housing insecurity, or other disruptive event that affected your academic standing.
Note: This is not a comprehensive list. If your situation is not listed here but you believe it significantly impacted your scholarship eligibility, you are still encouraged to submit a reinstatement request with a detailed explanation.
Important notes
- Only complete Reinstatement Requests will be reviewed. Please ensure all sections of the form are complete before submitting.
- All complete requests must include a personal statement and the completed Request. Examples of these are included on the Reinstatement form for your reference.
- Our office may require additional information upon review, so please check your NAU email regularly.
- Submitting this request is not a guarantee of reinstatement.
- Processing time begins once we receive your complete request. Requests are reviewed in the order that they are received. Please allow up to 25 business days for a decision to be sent to your NAU email.
- The deadlines are Nov. 1 for fall semester, and April 1 for spring semester.
