• Room reservation request – please print and fill out the room reservation request and return to the area supervisor.
• Performance emphasis application – this form to apply for the Performance Emphasis.
• Design & Technical emphasis application – use this form to apply for the Design & Technical Emphasis.
• Production Position Application – this form is used to apply for all Production Positions.
• Senior Capstone proposal application – this form must be used to apply for a Senior Capstone project with the Department of Theatre.
• Theatre scholarship application – use this form to apply for a Department of Theatre scholarship.

• NAU registrar override form – use this form to apply for an override to get into classes that may require faculty approval. TH 113 & TH 440 are examples.
