“Exploring Unmet Rehabilitation Needs in American Indians through Community Partnership”

NAU Academic Partners: Amit Kumar, PhD, MPH; Assistant Professor, Department of Physical Therapy, and Alyssa Concha-Chavez, OTD, OTR/L; Program Coordinator, NAU Community Health Mentor Program

Community Partner: Jon Cook, DPT, PT; Director of Rehabilitation Services, Flagstaff Medical Center

Goal of Partnership: Work in partnership with the Northern Arizona Healthcare System to understand the unmet rehabilitation needs of American Indians living in rural areas for the larger goal of improving rehabilitation practice and delivery reflective of community preferences.


“Using Community Networking Opportunities and Implementation Science Education to Improve Supports and Collaboration for Foster Care in Arizona”

NAU Academic Partner: Amy Armstrong-Heimsoth, OTD, OTR/L; Assistant Clinical Professor, Department of Occupational Therapy

Community Partner: Shevaun Sullivan; Program Manager; Opportunity, Community and Justice for Kids (OCJ Kids), Phoenix

Goal of Partnership: Improve communication, collaboration and networking opportunities among entities providing foster care youth services and support, as well as the capacity of local organizations to utilize the principles of implementation science in program development.
